If you would like to start using online services for yourself, or if you would like to grant a proxy user, e.g., a carer or relative, to enable you to view your GP records, make appointments and order repeat prescriptions online, please complete the Patient Access forms at the surgery. You will need to bring along your ID documents; proof of photo ID and address.

Proof of your new address is desirable. This can be in the form of a utility bill, bank statement or driving licence etc. Please bring this with you when you visit to complete your registration.
You should visit the surgery, to show ID, within one month of completing the online application (in Step 1 above). After this time your application will no longer be valid and should be re-submitted.
If you have a medical card please bring it with you to the surgery as will assist us in transferring your existing medical records (though cards have now been phased out, so many patients will not have one).
You will also be given a selection of further information and a practice leaflet which gives you all the information about Vine Surgery, the services provided, and opening times.
