COMMUNITY TEAM
Community Nurses
The District Nursing team look after the healthcare needs of patients who are housebound or have a life-threatening illness in their own homes. They work in collaboration with the doctors, specialist nurses, hospices, social services, and voluntary organisations to provide a high quality holistic care service for patients and carers. They also carry out Social Services assessments and funded nursing care assessment. The district nurses are happy to answer any queries you may have about community health care issues. If they don't know the answer they'll point you in the right direction. The district nursing team are available every Monday to Friday 8 am - 6 pm, telephone 0300 124 5602 or call NHS 111 after 6pm and before 8am and at the weekend.
Health Visitors
Health Care Studies are allocated by Mendip PCT to work with named doctors, and are assigned a caseload. Health Visitors are qualified nurses with special training and experience in child health, health promotion and education. They are responsible, with the nursing staff, for the health and welfare of the young patients registered with our practice. They give information and advice to parents about the prevention of ill health, child development, immunisations parent craft and parenting issues affecting the family. The Health Visitors run our "well baby" clinics, post-natal support groups and eneuretic clinic. They will soon be implementing a hearing-screening programme for newborn babies. You can telephone our Health Visitors on 01458 840552.
Community Midwife
Midwives look after our patients throughout pregnancy, mainly providing ante-natal, and post-natal care. They occasionally provide intra-partum care for home births. The ante-natal care is mainly through clinics at Vine Surgery Partnership by appointment. Postnatal care is up to 10 days in the home and according to mothers' needs. At 10 days care passes to Health Visitors. Messages can be left on the Midwives' answerphone 01749 836687. Emergency advice can be obtained on 01823 342571.
